Working hand in hand with existing electronic health records (EHRs) and case management systems, P-CIS helps evidence program value by converting assessment data into visual stories of progress over time. Staff, clinicians, peer helpers, family teams and individuals can visually track progress along goals to address needs, celebrate strengths and skills and sensitively note adverse events and history. P-CIS automates the implementation of Level of Care, presenting recommendations as soon as information is gathered. It also supports access (with approval) to relevant information for all of the diverse members of an individual’s or family’s care and support team – facilitating person-centered coordination of care.
Focusing on staff and program strengths, P-CIS helps match the right individual to the right staff and program to support the best possible outcomes. Because P-CIS organizes information in easy-to-read visualizations which can be quickly shared and interpreted by the entire care team, consumers will not need to tell and retell their story.
